feat: implement lazy loading for CLI startup optimization

Optimize startup time by deferring heavy module imports until actually needed:

**Tool Module Loading (Primary Impact)**
- Move eager imports of 17 tool modules into lazy _ensure_tools_loaded() function
- Tools now load only when get_registered_tools() is called
- Implements __getattr__ for backward-compatible lazy class imports
- Estimated savings: 100-200ms on systems with slower I/O

**Command App Loading (Structural Improvement)**
- Replace direct imports of MCP/Skill/Plugin/Prompt/Agent apps with lazy wrappers
- Add caching mechanism to load each app only once
- Maintain full CLI functionality while improving import structure
- Foundation for future true command-level lazy loading

**New Module: lazy_loader.py**
- Provides reusable lazy-loading utilities:
  - LazyModule: Proxy for entire modules
  - LazyFunction: Wrapper for functions
  - LazyAttribute: Wrapper for attributes
- Clean, extensible foundation for future optimizations

**Testing & Compatibility**
- All core CLI tests pass (34/34)
- Backward compatibility maintained for direct tool imports
- No breaking changes to public API
- Help output and all subcommands work correctly

+### 1. Created New Lazy Loading Utilities Module
+**File:** `code_assistant_manager/lazy_loader.py`
+
+Provides utilities for deferred module imports:
+- `LazyModule`: Lazy-loading proxy for entire modules
+- `LazyFunction`: Lazy-loading wrapper for functions
+- `LazyAttribute`: Lazy-loading wrapper for attributes
+- Helper functions: `lazy_import()`, `lazy_function()`, `lazy_attr()`
+- `preload_tools()`: Explicitly load all tools when needed
+
+### 2. Refactored Tools Package Initialization
+**File:** `code_assistant_manager/tools/__init__.py`
+
+Key changes:
+- **Removed** eager imports of all 17 tool modules (claude, copilot, codex, qwen, etc.)
+- **Added** `_ensure_tools_loaded()` function that lazy-loads tools only when `get_registered_tools()` is called
+- **Implemented** `__getattr__` module-level function for backward-compatible lazy loading of tool classes

+   - Could switch to Click's `lazy_group` for complete lazy loading

+   - ConfigManager could be lazy-loaded for commands that don't need it
+   - Would save additional time for quick help output
